This is a fresh option registration round — candidates who received an allotment in phases 1 or 2 but did not join, or who received no allotment, must register their course and college preferences again to be considered in this round. The registration window is short: it opens on 28 September and closes on 7 October at 10:00 AM. Allotments will be published on 8 and 9 October, and joining must happen between 10 and 14 October. If you miss the registration deadline or fail to join by the prescribed date, you will lose all previous allotments and cannot participate further.

What is the third phase and who must register?
The third phase is the final round of state-quota MBBS and BDS seat allotment in Kerala for 2026–27. It is open to all candidates whose names appear in the official Medical rank lists published by CEE.
You MUST register fresh options in this phase if: (1) you received an allotment in phase 1 or 2 but did not take admission, or took a Transfer Certificate (TC) after joining; or (2) you received no allotment in phases 1 or 2. If you do not register, you will not be considered for this round, and any previous allotment will be retained (you cannot change it).
If you already joined a college in phase 1 or 2 and are happy with your seat, you do NOT need to register again — your admission is safe. However, if you wish to vacate that seat and try for a different college or course, you can exit by forfeiting your registration fee before 7 October 10:00 AM.
Important dates and timeline
The entire third phase process runs over two weeks. Mark these dates carefully — missing any deadline will cost you your seat.
Registration opens on 28 September and closes on 7 October at 10:00 AM sharp. After 10:00 AM on 7 October, the portal will close and no further registrations will be accepted. Provisional allotment results are published on 8 October, and final allotment on 9 October. You must join your allotted college between 10 and 14 October by 4:00 PM. Colleges will then submit the list of admitted candidates by 5:00 PM on 14 October.

How to register your options
Registration is entirely online. Visit www.cee.kerala.gov.in from 28 September onwards. Log in with your credentials (the same ones you used for earlier phases). You will see a form asking you to list your preferred colleges and courses in order of preference.
Register only for colleges and courses you are genuinely willing to join. If you are allotted a seat and do not join by 14 October 4:00 PM, you will lose that allotment AND all your previous allotments in phases 1 and 2, and you will be barred from any further counselling in this stream. This is a permanent loss — there is no appeal or second chance.
After you submit your preferences, you will receive a confirmation. Keep a record of your registration number and the options you submitted. You can check the portal again to verify your registration was successful.

Registration fee: who pays what
Most candidates registering afresh must pay a registration fee of ₹5,000 (Rupees Five Thousand). This fee is non-refundable if you do not join your allotted college, but it will be adjusted or refunded in certain cases (see below). Payment is online only — no cash or cheque accepted.
Candidates belonging to SC, ST, or OEC (Other Eligible Communities) categories, and those with benefits under Government Orders G.O.(Ms) No.25/2005/SCSTDD (dated 20.06.2005) or G.O.(Ms) No.10/2014/BCDD (dated 23.05.2014), pay only ₹500 (Rupees Five Hundred). Candidates from Juvenile Home, Nirbhaya Home, or Sree Chitra Home also pay ₹500. These reduced fees will be adjusted in the caution deposit if you join a college.
If you are allotted a seat in a Government Medical or Dental College, your registration fee will be carried over and adjusted against your tuition fee. If you are allotted a seat in a Private Self-Financing Medical or Dental College and you join, your registration fee will be refunded after the closing date of admission. If you do not get any allotment, your registration fee will be refunded. If you pay the fee but do not join your allotted college, the fee is treated as a penalty and will NOT be refunded.

Course fees and token fees
The full fee structure for MBBS and BDS courses at all colleges is available on the CEE website (www.cee.kerala.gov.in). For Government Medical and Dental Colleges, fees are fixed by the state. For Private Self-Financing colleges, fees vary by institution.
Important: The 2026–27 fee for Private Self-Financing Medical and Dental Colleges has not yet been finalized. If you are allotted a seat in a Private college, you must provisionally pay the fee that was charged in 2025–26. If the 2026–27 fee (to be set by the Admission Supervisory Committee, Government, or High Court) is higher, you will have to pay the difference. If it is lower, you will receive a refund.
After you receive your allotment, you must pay a 'token fee' (the full course fee or balance fee) to secure your seat. For Private colleges, pay the fee directly to the college as shown in your allotment memo. For Government colleges, pay the complete fee to CEE. Candidates in SC/ST/OEC/Fishermen categories and those from special homes do NOT pay a token fee — EXCEPT if they are allotted under Minority or NRI quota seats in Private colleges, in which case they must pay the full fee with no concession.

Critical trap: what happens if you don't join by the deadline
This is the most important rule to understand. If you are allotted a seat in the third phase and do NOT join the college by 14 October 4:00 PM, you will lose THREE things: (1) your allotment in phase 3, (2) your allotment in phase 2 (if you had one), and (3) all your options in the stream to which the allotment belongs. You will be permanently barred from further counselling in that stream.
This rule applies even if you have a valid reason for not joining (illness, travel, etc.). There is no grace period, no appeal, and no exception. Once the deadline passes, your seat is gone and will be offered to the next candidate. Therefore, register only for colleges and courses you are absolutely certain you will join.
If you wish to vacate a seat you joined in phase 1 or 2 and try for a different college in phase 3, you must exit before 7 October 10:00 AM by forfeiting your registration fee. After that, you cannot exit without losing all allotments.

Special rule: All India Quota (AIQ) candidates
If you were allotted a seat under the All India Quota (AIQ) in phase 3 through the Medical Counselling Committee (MCC), you may become ineligible for Kerala's state-quota phase 3 counselling. The MCC will issue a directive and share a list of such candidates with CEE. If your name appears on that list, your options for the state-quota third phase will NOT be considered, and you will be ineligible to participate in the state round.
This rule prevents double allotments — you cannot hold both an AIQ seat and a state-quota seat. If you are unsure whether you are affected, check the CEE website or contact the helpline.

Unallotted category seats and stray vacancy allotment
During the third phase, any category-specific seats (e.g., SC, ST, OEC, Fishermen, Minority, NRI) that remain unallotted will be converted to general seats and filled from the general merit list. This is done automatically as per the prospectus rules.
After the third phase allotment is complete, if any MBBS or BDS seats remain vacant in any college or category, they will be filled through a Stray Vacancy Allotment process. Details of the Stray Vacancy Allotment will be published later on the CEE website. This is your last chance to secure a seat if you were not allotted in the main three phases.
