The Tamil Nadu Directorate of Medical Education & Research (DME), Chennai, has released the official fee structure for postgraduate medical courses (MD, MS, and PG Diploma) for the 2026-27 academic session. This notice, issued on 30 October 2026, sets out the exact fees you will pay based on your quota (Government, Management, or NRI) and course type (clinical degree, non-clinical degree, or diploma). Understanding these fees is essential before you participate in NEET PG counselling, as they determine your financial commitment and help you plan your admission strategy. The notice also clarifies fee exemptions for differently abled candidates in government institutions.

Government Quota PG MD/MS fees are ₹3–3.5 lakh per annum; Management Quota ranges from ₹5–16 lakh; NRI Quota from ₹19–29 lakh — and differently abled candidates are exempt from tuition and special fees in government colleges.

What This Notice Covers

This is the official fee schedule for all postgraduate medical courses offered in Tamil Nadu government and private (management quota) medical colleges for the 2026-27 session. It applies to MD degrees (Doctor of Medicine), MS degrees (Master of Surgery), and PG Diploma courses in both clinical and non-clinical specialties. The fees are fixed by the Tamil Nadu government and are binding on all participating institutions.

The notice distinguishes between three quota categories — Government Quota (seats filled by the state), Management Quota (seats managed by private colleges), and NRI Quota (seats for Non-Resident Indian candidates). Each quota has its own fee structure, and the fees vary depending on whether your course is a clinical specialty (e.g., Medicine, Surgery, Pediatrics) or a non-clinical specialty (e.g., Pathology, Microbiology, Biochemistry).

Fee Structure by Quota and Course Type

The fees are presented in three tiers based on the quota you secure during NEET PG counselling. Government Quota seats carry the lowest fees, Management Quota seats are significantly higher, and NRI Quota seats are the most expensive. Within each quota, clinical degrees (MD/MS in clinical subjects) are more expensive than non-clinical degrees and diplomas.

It is important to note that these are annual fees — you will pay this amount each year for the duration of your course. For a 3-year MD/MS program, you will pay the annual fee three times. For a 2-year diploma, you will pay twice. Always multiply the annual fee by the course duration when calculating your total financial commitment.

Government Quota Fees in Government Colleges

If you secure a Government Quota seat in a government medical college, your annual fee is fixed at ₹50,000 for MD/MS degrees and ₹20,000 for PG Diploma courses. These are significantly lower than the fees shown in the main table above, which apply to private (management quota) institutions. This is a major financial advantage of securing a government seat.

These government fees are set by Government Order (G.O.) No. 630 from the Health and Family Welfare Department (dated 12 June 2023) for MD/MS degrees, and by G.O. No. 135 from the Social Welfare and Nutritious Meal Programme Department (dated 20 September 2008) for diploma courses. The fees are fixed per annum and apply uniformly across all government medical colleges in Tamil Nadu.

Clinical vs. Non-Clinical Specialties — Fee Difference

Clinical specialties (such as General Medicine, General Surgery, Pediatrics, Obstetrics & Gynaecology, Psychiatry, Radiology, Anesthesia) are more expensive than non-clinical specialties (such as Pathology, Microbiology, Biochemistry, Pharmacology, Forensic Medicine, Community Medicine). In Government Quota seats, clinical degrees cost ₹3.5 lakh per annum, while non-clinical degrees cost ₹3 lakh per annum — a difference of ₹50,000 per year.

In Management Quota seats, the gap is even larger: clinical degrees cost ₹16 lakh per annum, while non-clinical degrees cost ₹5 lakh per annum. Similarly, in NRI Quota, clinical degrees are ₹29 lakh per annum versus ₹19 lakh for non-clinical. When choosing your specialties during counselling, factor in this fee difference alongside your career preferences.

NRI Quota Fees — What Non-Resident Indians Must Pay

If you are a Non-Resident Indian (NRI) candidate and secure an NRI Quota seat, you will pay significantly higher fees than Government or Management Quota candidates. NRI Quota fees range from ₹19 lakh per annum (non-clinical) to ₹29 lakh per annum (clinical). These fees apply regardless of whether your seat is in a government or private college.

NRI status is determined by your passport and residency status as per the Reserve Bank of India (RBI) definition. If you hold an Indian passport but are a resident of another country, or if you are an Overseas Citizen of India (OCI), you may be classified as NRI. Confirm your NRI eligibility with the counselling authority before the counselling process begins, as this affects both your seat allocation and your fees.

Fee Exemption for Differently Abled Candidates

Eligible differently abled (PwD — Person with Disability) candidates are granted a complete exemption from tuition fees and special fees during NEET PG counselling and admission in government medical institutions. This exemption applies to both MD/MS degrees and PG Diploma courses. The exemption is based on Government Orders from the Social Welfare and Nutritious Meal Programme Department (G.O. No. 135, dated 20 September 2008) and the Welfare of Differently Abled Persons Department (G.O. No. 30, dated 28 June 2010).

To claim this exemption, you must have a valid disability certificate issued by a competent medical authority and must be recognized as eligible under the state's disability welfare scheme. You will need to produce this certificate during the counselling process. The exemption covers tuition fees and special fees only — other charges (such as hostel fees, examination fees, or development fees) may still apply depending on the college's rules. Contact the DME office or your college directly to clarify which fees are covered under the exemption.

  • Exemption applies to tuition fees and special fees only
  • Available in government medical institutions only
  • Requires a valid disability certificate from a competent authority
  • Applies to both MD/MS and PG Diploma courses

When and How to Pay These Fees

Fees are typically paid after you are allotted a seat during NEET PG counselling and you have accepted the seat. The exact payment schedule, payment gateway, and due dates will be communicated by the counselling authority (Tamil Nadu CET Cell or DME) in a separate notification. Do not assume payment is due immediately after allotment — wait for the official payment schedule.

Important: These Are Annual Fees, Not Total Course Fees

A critical point that many candidates miss: the fees listed in this notice are annual fees. If you are admitted to a 3-year MD or MS program, you will pay the annual fee three times — once each year. If you are admitted to a 2-year diploma, you will pay twice. For example, if you secure a Government Quota seat in a clinical MD program, you will pay ₹3.5 lakh in Year 1, ₹3.5 lakh in Year 2, and ₹3.5 lakh in Year 3, for a total of ₹10.5 lakh over three years.

When planning your finances or taking an education loan, always calculate the total cost by multiplying the annual fee by the course duration. Do not confuse the annual fee with the total course fee.

  • MD/MS programs are typically 3 years — multiply annual fee by 3
  • PG Diploma programs are typically 2 years — multiply annual fee by 2
  • Example: Government Quota clinical MD = ₹3.5 lakh × 3 years = ₹10.5 lakh total

Management Quota vs. Government Quota — The Cost Difference

The fee difference between Government Quota and Management Quota seats is substantial. For a clinical MD degree, Government Quota costs ₹3.5 lakh per annum, while Management Quota costs ₹16 lakh per annum — more than four times higher. Over a 3-year program, this is a difference of ₹37.5 lakh. This is why many candidates prioritize government seats during counselling, even if it means accepting a less preferred specialty.

However, Management Quota seats may offer advantages such as better infrastructure, more flexible course schedules, or proximity to your home. Weigh the financial burden against these factors when deciding which seats to choose during counselling. Remember that you can only accept one seat, so choose carefully based on your financial capacity and career goals.

Frequently asked questions

I secured a Government Quota seat in a clinical MD program. How much will I pay in total?

You will pay ₹50,000 per annum as per the government fee structure. For a 3-year MD program, your total fee will be ₹1.5 lakh (₹50,000 × 3 years). This is significantly lower than the ₹3.5 lakh per annum shown in the main table, which applies only to private (management quota) institutions.

What is the difference between the fees in the main table and the government fees mentioned separately?

The main table shows fees for Management Quota seats (private colleges) and NRI Quota seats. The separate section shows fees for Government Quota seats in government colleges, which are much lower. If you secure a Government Quota seat, you pay the lower government fee (₹50,000 for MD/MS, ₹20,000 for diploma), not the higher fees in the main table.

I am a differently abled candidate. Do I have to pay any fees?

Eligible differently abled candidates are exempt from tuition fees and special fees in government medical institutions. However, you must have a valid disability certificate from a competent authority. Other charges such as hostel fees or examination fees may still apply — contact your college for details. This exemption applies only in government colleges, not in management quota seats.

I am an NRI. How much will I pay if I get a seat?

NRI Quota fees are ₹19 lakh per annum for non-clinical courses and ₹29 lakh per annum for clinical courses. These are annual fees, so for a 3-year MD program in a clinical specialty, you will pay ₹29 lakh × 3 = ₹87 lakh in total. NRI fees apply regardless of whether your seat is in a government or private college.

Are clinical specialties always more expensive than non-clinical?

Yes. In Government Quota, clinical degrees cost ₹3.5 lakh per annum while non-clinical cost ₹3 lakh. In Management Quota, clinical costs ₹16 lakh while non-clinical costs ₹5 lakh. In NRI Quota, clinical costs ₹29 lakh while non-clinical costs ₹19 lakh. The fee difference is significant, so factor this into your specialty choices during counselling.

When do I have to pay these fees?

Fees are paid after you are allotted a seat during NEET PG counselling and you have accepted the seat. The exact payment dates and schedule will be notified separately by the Tamil Nadu CET Cell or DME. Do not pay before receiving an official payment notification, as you may lose your money if you do not secure a seat.

Are there any other fees I need to pay besides the course fees listed in this notice?

Yes. This notice covers tuition fees only. Colleges may also charge hostel fees, examination fees, library fees, development fees, and activity fees, which vary by institution. Additionally, the counselling authority may charge a registration fee during the admission process. Contact your allotted college for a complete fee breakdown.

I am planning to take an education loan. What total amount should I calculate?

Multiply the annual fee by the course duration. For example, if you secure a Management Quota seat in a clinical MD program, calculate ₹16 lakh × 3 years = ₹48 lakh for course fees alone. Add hostel fees (if applicable) and other charges for the total amount. Discuss the exact breakdown with your college before finalizing the loan.

Do Government Quota fees apply in private colleges?

No. Government Quota fees (₹50,000 for MD/MS, ₹20,000 for diploma) apply only in government medical colleges. If you secure a Management Quota seat in a private college, you will pay the higher Management Quota fees shown in the main table (₹3–3.5 lakh for MD/MS, ₹3 lakh for diploma).

What if I cannot pay the fees by the deadline?

If you fail to pay fees by the deadline set by the counselling authority, your seat will be cancelled and offered to the next candidate on the merit list. You will lose your admission. Plan your finances in advance and ensure you can pay on time. If you face genuine hardship, contact the college or DME office immediately to request an extension, though this is not guaranteed.

This fee structure is now official for the 2026-27 session. Before you participate in NEET PG counselling, review these fees carefully and calculate your total financial commitment based on the course duration and quota you are likely to secure. If you are a differently abled candidate, gather your disability certificate now so you can claim the exemption during counselling. Keep this notice handy during the counselling process, and watch for the official payment schedule notification from the Tamil Nadu CET Cell. If you have questions about fees or exemptions, contact the DME office in Chennai or your allotted college directly.
