The Directorate of Medical Education (DME), Chhattisgarh, has released the official certificate formats that NEET UG 2026 candidates must use when submitting supporting documents during counselling and admission. These templates cover domicile certificates, category certificates (Sainik Varg A, B, C), Freedom Fighter certificates, PH (Persons with Disability) certificates, and the non-judicial certificates required for final admission to medical and dental colleges. Using the exact format specified by DME is critical — certificates in any other format will be rejected during document verification, potentially costing you your seat. This notice provides downloadable PDF templates for each certificate type so you can ensure your documents match the official requirements.
Download and use ONLY the official Chhattisgarh DME certificate formats for your category and college type — any other format will result in rejection during admission verification.

During NEET UG 2026 counselling and admission in Chhattisgarh, you will be asked to submit supporting certificates to prove your eligibility for reserved seats, domicile status, or special categories. The DME has now published the EXACT formats these certificates must follow. If your certificate does not match the official template — even if the information is correct — it will be rejected at document verification, and you will lose your seat.
This is not optional guidance. These are the mandatory formats. Your school, college, or issuing authority must issue your certificate in the exact layout, wording, and structure shown in the DME's PDF templates. If they refuse or issue it in a different format, you must request them to reissue it in the correct format before you submit it during counselling.
Which Certificates Are Covered
The DME has released official formats for seven types of certificates. Each serves a different purpose in the admission process. You need to download and use the format that matches your situation — if you belong to multiple categories, you may need more than one certificate.
- Medical Board Certificate for PH (Persons with Disability) — if you are applying under the PH quota
- Freedom Fighter Certificate — if you are a Freedom Fighter or the child/grandchild of a Freedom Fighter
- CG Domicile Certificate — if you need to prove you are a Chhattisgarh resident (required for state quota seats)
- Sainik Varg Certificate C — for ex-servicemen and their dependents (lowest priority in Sainik Varg)
- Sainik Varg Certificate B — for ex-servicemen and their dependents (middle priority)
- Sainik Varg Certificate A — for ex-servicemen and their dependents (highest priority)
- Non-Judicial Certificate for Medical College — required at final admission to a medical college
- Non-Judicial Certificate for Dental College — required at final admission to a dental college
Important: Domicile Certificate Format
If you are applying for Chhattisgarh state quota seats (which are reserved for Chhattisgarh residents), you MUST submit a domicile certificate in the official CG Domicile Certificate format. This certificate proves that you have lived in Chhattisgarh for the required period. Without it, you cannot be considered for state quota seats, even if you were born in Chhattisgarh.
The domicile certificate must be issued by the Collector or Sub-Divisional Officer (SDO) of your district. When you apply for it, provide them with the official DME format PDF so they issue it correctly. Many candidates lose seats because their domicile certificate is in the wrong format or lacks required details.

Category Certificates: Sainik Varg (Ex-Servicemen)
If you are the child or dependent of an ex-serviceman, you are eligible for Sainik Varg (military service) category seats. Chhattisgarh recognizes three sub-categories within Sainik Varg, each with a different priority level. You must obtain the certificate for the correct sub-category that applies to your situation.
Sainik Varg Certificate A is for ex-servicemen who were disabled in service or died in service, or their dependents — this has the highest priority. Sainik Varg Certificate B is for other ex-servicemen and their dependents. Sainik Varg Certificate C is for a third category of ex-servicemen dependents. Ask your issuing authority (usually the District Administration or Military Records Office) which certificate type you qualify for, and provide them with the correct DME format template.
PH (Persons with Disability) Certificate Format
If you have a disability and are applying under the PH quota, you must submit a Medical Board Certificate for PH in the official format. This certificate must be issued by a Medical Board authorized by the state government, not by a private doctor or a general hospital. The certificate must clearly state the type and percentage of disability.
Ensure the Medical Board uses the exact DME format template. If your certificate is issued in any other format, it will be rejected, and you will not be considered for PH quota seats. If you already have a disability certificate from another state or authority, it may not be accepted — you may need to obtain a fresh certificate from a Chhattisgarh Medical Board in the official format.
Freedom Fighter Certificate
If you are a Freedom Fighter or a direct descendant (child or grandchild) of a Freedom Fighter, you may be eligible for reserved seats under the Freedom Fighter category. You must submit a Freedom Fighter Certificate in the official DME format. This certificate is typically issued by the District Administration or the office of the Collector.
Contact your District Administration well in advance to apply for this certificate. Provide them with the official DME format template and any supporting documents (such as your ancestor's Freedom Fighter recognition certificate or government records). Processing can take time, so do not wait until the last moment.
Non-Judicial Certificates for Final Admission
After you are allotted a seat in counselling, you will be asked to report to the college for final admission. At that time, you must submit a Non-Judicial Certificate (also called a Bonafide Certificate or Character Certificate). This is a formal document issued by your school or college confirming that you are a genuine student and have no criminal record.
Chhattisgarh DME has released TWO separate formats: one for candidates admitted to medical colleges and one for candidates admitted to dental colleges. You must use the correct format for your college type. If you are admitted to a medical college, use the Medical College format; if admitted to a dental college, use the Dental College format. Ask your school or college to issue this certificate in the exact format specified by DME.

How to Download the Certificate Formats
All seven certificate format templates are available on the Chhattisgarh DME website at cgdme.admissions.nic.in/certificate-format/. Each template is provided as a PDF file that you can view and download. The file sizes range from 82 KB to 685 KB.
Download the PDF for each certificate type you need. You can print it and show it to the issuing authority (your school, the District Administration, the Medical Board, etc.) so they know exactly how to format your certificate. Alternatively, you can email the PDF to the issuing authority or provide it digitally. Keep a copy for your own records.
Common Mistakes That Get Certificates Rejected
Many candidates lose their seats because their certificates are rejected during document verification. Here are the most common reasons:
- Using a certificate in a different format — even if the information is correct, if the layout or wording does not match the official template, it will be rejected.
- Missing required fields — the official format includes specific fields that must be filled in. If any field is left blank or incomplete, the certificate is invalid.
- Wrong certificate type — submitting a Sainik Varg Certificate B when you should have submitted Certificate A, or vice versa.
- Issuing authority not authorized — for example, submitting a disability certificate from a private doctor instead of an authorized Medical Board.
- Certificate issued after the deadline — some certificates must be issued on or after a specific date. Check the DME's counselling notification for any date restrictions.
- Domicile certificate from the wrong authority — a domicile certificate must be issued by the Collector or SDO, not by a Tehsildar or other lower authority.
- Non-Judicial Certificate in the wrong format — using the Medical College format when you are admitted to a dental college, or vice versa.
When to Obtain These Certificates
Do not wait until counselling begins to apply for these certificates. Start the process NOW. Obtaining certificates from government authorities can take weeks or even months, especially if there are delays or if documents are incomplete.
For domicile certificates, contact your District Collector's office or SDO immediately. For Sainik Varg certificates, contact the District Administration or Military Records Office. For PH certificates, contact an authorized Medical Board. For Freedom Fighter certificates, contact the District Administration. For Non-Judicial Certificates, contact your school or college. Provide each authority with the official DME format template so they issue it correctly on the first attempt.
What to Do If Your Issuing Authority Refuses to Use the Official Format
If the authority issuing your certificate (your school, the District Administration, the Medical Board, etc.) refuses to use the official DME format or says they have their own format, escalate the issue. Contact the DME directly or file a complaint with the District Administration. The DME's official format is mandatory for Chhattisgarh NEET UG admissions, and any authority refusing to follow it is not complying with state regulations.
Do not accept a certificate in any other format. If you submit a non-compliant certificate during counselling, it will be rejected, and you will have no recourse. It is better to spend time now ensuring the certificate is issued correctly than to lose your seat later.

See My CollegesFrequently asked questions
What happens if I submit a certificate that does not match the official DME format?
Your certificate will be rejected during document verification at the college. You will not be able to complete your admission, and you will lose your seat. The DME formats are mandatory — there are no exceptions. Always use the official template.
Where do I download the official certificate formats?
All seven certificate formats are available on the Chhattisgarh DME website at cgdme.admissions.nic.in/certificate-format/. Each is provided as a downloadable PDF file. Download the format(s) you need and provide them to the issuing authority.
I belong to multiple categories (e.g., I am both PH and Sainik Varg). Do I need multiple certificates?
Yes, you may need to submit certificates for each category you are eligible for. Download the format for each category and obtain the certificates from the appropriate authorities. During counselling, you will declare which category you want to apply under, and you will submit the relevant certificate(s).
Who issues the domicile certificate, and how long does it take?
The domicile certificate is issued by the Collector or Sub-Divisional Officer (SDO) of your district. Processing time varies, but it can take 2-4 weeks or longer. Apply immediately and provide the official DME format template to speed up the process.
I have a disability certificate from another state. Can I use it for Chhattisgarh NEET UG admission?
No. You must obtain a fresh PH (Persons with Disability) certificate from an authorized Medical Board in Chhattisgarh, issued in the official DME format. Certificates from other states or authorities will not be accepted.
What is the difference between Sainik Varg Certificate A, B, and C?
These are three sub-categories within the Sainik Varg (ex-servicemen) quota. Certificate A is for ex-servicemen disabled in service or their dependents (highest priority). Certificate B is for other ex-servicemen and dependents. Certificate C is for a third category. Ask your issuing authority which certificate type you qualify for based on your situation.
When do I need to submit the Non-Judicial Certificate?
You submit the Non-Judicial Certificate at the time of final admission to the college, after you have been allotted a seat in counselling. Use the Medical College format if you are admitted to a medical college, or the Dental College format if admitted to a dental college.
Can my school issue the Non-Judicial Certificate in any format, or must it follow the DME template?
It must follow the official DME template. Provide your school with the correct format PDF (Medical College or Dental College, depending on where you are admitted) and ask them to issue the certificate in that exact format. If they refuse, escalate to the DME.
I am a Freedom Fighter's grandchild. Am I eligible for the Freedom Fighter category?
Yes, direct descendants (children and grandchildren) of Freedom Fighters are eligible. You must obtain a Freedom Fighter Certificate from the District Administration in the official DME format. Provide supporting documents such as your ancestor's Freedom Fighter recognition certificate or government records.
What if I apply for a certificate now but the issuing authority makes a mistake in the format?
If the certificate is issued incorrectly, you must request the authority to reissue it in the correct format. Do not wait until counselling to discover the error. Verify the certificate matches the official DME template before you submit it during admission. If there is an error, contact the issuing authority immediately for a corrected version.
Download the official certificate formats from cgdme.admissions.nic.in/certificate-format/ immediately. Identify which certificates you need based on your category and eligibility. Contact the issuing authorities (your school, District Administration, Medical Board, etc.) NOW and provide them with the official DME format templates. Do not wait — obtaining government certificates can take weeks. Verify that each certificate matches the official format before you submit it during counselling. Any certificate not in the official format will be rejected, and you will lose your seat.
