The office of the Chairman, NEET UG Admission/Counselling Board 2026, at SMS Medical College, Jaipur, has issued the checklist of documents every candidate allotted an MBBS or BDS seat in Rajasthan must bring at the time of reporting/joining. This is not a new application or a result — it is a mandatory checklist that tells allotted candidates exactly what to carry to the college reporting desk, along with a signed undertaking (Proforma-7) confirming that all documents are genuine originals. Candidates must also carry printed and preserved copies of their online application form. The notice is important for anyone who gets a Rajasthan MBBS/BDS seat allotment in NEET UG 2026 counselling, because incomplete or invalid documents can lead to cancellation of the seat.
Every candidate allotted a seat must appear at the reporting desk with all listed original documents, photocopies, photographs and a signed Proforma-7 undertaking — missing even one required document, or carrying an expired category/EWS certificate, can get the admission rejected on the spot.
What this notice actually is
This document is titled 'List of Documents Needed at Time of Reporting/Joining at College Desk at Academic Block, SMS Medical College, Jaipur along with Application Form & Undertaking (Proforma-7)'. It is issued by the office of the Chairman of the NEET UG Admission/Counselling Board 2026, which runs Rajasthan's state counselling for MBBS and BDS seats.
It applies to any candidate who has received a seat allotment through Rajasthan NEET UG 2026 counselling and is proceeding to report/join the allotted medical or dental college. It does not open any new application window and does not announce any counselling round dates — it is purely a document-readiness checklist for the reporting stage.
Print and preserve: keep 2 copies of your online application form
The notice instructs candidates to print and safely keep 2 copies of the application form filled online, along with every document listed below, in original.

Extra items for NRI-quota candidates
Candidates joining against NRI (Non-Resident Indian) seats must submit additional proofs about their sponsor:
⚠️ Trap to watch: the NRI undertaking (whether Proforma-3 or Proforma-4) must be issued ON OR AFTER the date of the NEET 2026 examination, i.e. 21 June 2026. An undertaking dated before this will not be accepted.
- If the sponsor NRI/OCI/PIO currently resides in India: a certificate from the Indian embassy in the concerned country, or from the Ministry of External Affairs, Government of India, PLUS an undertaking in Proforma-3 on non-judicial stamp paper of Rs. 50, executed by the sponsor and duly notarised.
- OR, if the sponsor resides outside India: a duly notarised undertaking in Proforma-4, executed by the sponsor and notarised by a Notary Public in the sponsor's country of residence.
- Father/Mother/Guardian must also submit an undertaking in Proforma-5, stating the relationship between the sponsor and the candidate, along with the address of the parents and the local (Indian) guardian.
- Address proof of both the parents and the local (Indian) guardian.
What else to carry at reporting — photos, photocopies, extra allotment letter copies
Besides the original documents above, candidates must physically carry the following to the reporting desk:
- 4 passport-size photographs — the same photograph that is affixed on the application form.
- 2 sets of self-attested photocopies of all the original documents.
- Copies of the allotment letter as required (the notice mentions both 6 copies for submission and 4 copies to be carried at reporting — carry at least 6 printed copies to be safe, since the requirement is stated more than once in the notice).
- A duly completed printed copy of the application form, to be submitted at reporting/joining at the specified place or the allotted college.
Proforma-7: the mandatory undertaking
Every candidate who has been allotted a seat must fill and sign Proforma-7, an undertaking submitted at the college desk. In this undertaking, the candidate declares:
If the candidate is below 18 years of age, the undertaking must also be signed by the father, mother or guardian.
- That they have received allotment for admission to an MBBS/BDS course in the named college.
- That they have deposited all required documents in original.
- That all the documents submitted are original and are not forged, photocopies or printed copies.
- That if any document is later found to not be original, or is found to be forged/photocopied/printed, the candidate will be responsible for cancellation of their admission and any further action taken by the appropriate authority.
Consequences of missing documents
The notice is explicit: if a candidate fails to submit the printed, duly completed application form at the time of reporting/joining — at the specified place or the allotted medical/dental college — their candidature will be rejected and they will be disqualified, without any further correspondence from the Board. This means there is no follow-up notice or reminder; the seat is simply lost.

See My CollegesFrequently asked questions
Do I need to carry photocopies as well as originals?
Yes. Along with all original documents, you must carry 2 sets of self-attested photocopies of every original document.
My OBC-NCL certificate is from last year — will it work?
No. The notice states that only OBC-NCL and MBC-NCL certificates valid for the year 2026-27 will be considered for reservation eligibility. An older certificate can get your reservation claim rejected.
Is a State EWS certificate from a previous year acceptable?
No. The State EWS certificate must specifically be valid for the year 2026-27.
I have an NRI-sponsor undertaking dated before the NEET exam — is that a problem?
Yes, it is a problem. The notice specifies the NRI undertaking must be issued on or after 21 June 2026, the date of the NEET UG 2026 examination. Get it re-issued/re-dated if it is older.
What happens if I forget to bring the printed application form on reporting day?
The notice states this leads to rejection of candidature/disqualification without further correspondence — so there is no second chance for this specific document.
How many photographs should I bring?
4 passport-size photographs, and they must be the same photograph you affixed on your application form.
Do I need a Date of Birth certificate separately from my 10th mark sheet?
No, your 10th class mark sheet/certificate (or any other equivalent DOB-showing certificate) itself serves as the Date of Birth proof.
Is Proforma-7 the same as the application form?
No. Proforma-7 is a separate undertaking, filled and signed only by candidates who have actually been allotted a seat, declaring that all submitted documents are genuine originals.
If you receive a Rajasthan NEET UG 2026 seat allotment, go through this checklist item by item well before your reporting date, and pay special attention to certificate validity years and the NRI undertaking date. Keep certified photocopies, extra allotment letter copies and passport photos ready in advance so nothing is missing at the reporting desk. For the exact reporting venue, dates and any category-specific formats referenced (Proforma 1 to 6), check the official counselling website and information bulletin.
