The Directorate of Medical Education (DME), Madhya Pradesh, has released the official proformas and affidavit templates that all NEET PG 2026 candidates must complete and submit during the admission process. These forms are mandatory for candidates seeking admission to postgraduate medical programmes (MD, MS, DNB, and PG Diploma) in both government and private medical colleges across Madhya Pradesh. The proformas cover candidate declarations, scrutiny committee verification, seat bond commitments, rural service obligations, and NRI sponsorship documentation. Understanding what each form requires and submitting them correctly is critical — missing or incomplete forms can lead to rejection of your admission.
All NEET PG 2026 candidates must complete and submit seven mandatory proformas and affidavits during admission; failure to do so will result in rejection of your seat allotment.

Unlock My College ListWhat Are These Forms and Why Do You Need Them?
The Madhya Pradesh DME has issued seven official proformas (forms) that every NEET PG 2026 candidate must fill out and submit at the time of admission to a medical college. These are not optional — they are a legal requirement under the Madhya Pradesh Medical Education Admission Rules, 2018, and all subsequent amendments (most recently updated on 30 September 2026).
These forms serve two purposes: (1) they confirm that you understand the rules, regulations, and conditions of your postgraduate programme, and (2) they create a binding legal record of your commitments — such as rural service obligations, seat bond amounts, and professional conduct standards. The scrutiny committee at your college will verify these forms before your admission is finalized.
The Seven Mandatory Proformas Explained
The DME has released seven distinct proformas, each serving a specific purpose in the admission process. Below is what each form covers and who must submit it.
- **Proforma-1 (Candidate Declaration & Document Checklist)**: This is the main declaration form you fill out. You list your NEET PG 2026 roll number, all-India merit rank, full name, parents' names, category (unreserved, SC, ST, OBC, EWS), disability status if applicable, mobile number, and email. You then tick boxes confirming you have read and understood the MP Medical Education Admission Rules 2018 and all amendments. You also declare that you are submitting original documents and that you understand the counselling process.
- **Proforma-2(a) (Seat Locking Bond – MD/MS State Quota)**: If you are allotted an MD or MS seat under the state quota (government college), you must sign this bond. It commits you to complete the full course. If you withdraw before the specified time limit or are dismissed, you must pay ₹30,00,000 (30 lakh rupees) to the college. The bond is secured by 0.5% of this amount (₹15,000) deposited upfront.
- **Proforma-2(b) (Rural Service Bond – MD/MS State Quota)**: If you are allotted an MD or MS seat under the state quota, you must also sign this rural service bond. For non-service candidates, you commit to 1 year of rural service after graduation; if you do not serve, you pay ₹10 lakh. For service candidates (already employed by the government), you commit to 5 years of rural service; if you do not serve, you pay ₹50 lakh. The bond is secured by 0.5% of the applicable amount deposited upfront.
- **Proforma-2(c) (Rural Service Bond – MD/MS All-India Quota)**: Similar to Proforma-2(b), but for candidates allotted seats under the all-India quota in government colleges. Non-service candidates commit to 1 year of rural service (₹10 lakh penalty if not served); service candidates commit to 5 years (₹50 lakh penalty). The bond is secured by 0.5% of the applicable amount.
- **Proforma-2(d) (Seat Locking & Rural Service Bond – Post-MBBS DNB/PG Diploma)**: For candidates allotted seats in post-MBBS DNB or PG Diploma programmes. The bond amount is ₹15,000 (for PG Diploma) or ₹25,000 (for DNB), secured by 0.5% of the bond value. Candidates commit to complete the course and serve in designated locations as per government rules.
- **Proforma-3 (NRI/OCI Candidate Affidavit)**: If you are an NRI (Non-Resident Indian) or OCI (Overseas Citizen of India) seeking admission under the NRI quota in a private medical college, you must submit this notarized affidavit on ₹1,000 non-judicial stamp paper. You declare your NRI/OCI status and provide proof (OCI card, NRI/OCI status certificate from embassy/consulate, or NRI passport copy). You also declare whether you are self-sponsored or sponsored by a relative (parent, grandparent, uncle/aunt, or sibling), and confirm that your sponsor will bear all fees.
- **Proforma-4 (Guardian/Sponsor Affidavit for NRI Candidates)**: If you are an NRI candidate sponsored by a guardian or relative, that person must submit this notarized affidavit on ₹1,000 non-judicial stamp paper. The sponsor declares their NRI/OCI status, provides proof, and confirms they will bear all your educational fees for the entire duration of the programme.
- **Proforma-5 (NRI/OCI Sponsorship Certificate)**: If you are sponsored by an NRI/OCI, the sponsor must submit this certificate, notarized in their country of residence. The sponsor declares their NRI/OCI status, provides their current address, confirms they have taken you as their ward, and undertakes to pay all your fees until you complete the programme.
Proforma-1: The Main Candidate Declaration Form
Proforma-1 is the foundation document. You must fill it out completely and accurately. It contains two parts: Part A (filled by you) and Part B (filled by the scrutiny committee at your college).
**Part A – What You Must Fill:**
In Part A, you declare that you have read and understood the MP Medical Education Admission Rules 2018 and all amendments (dated 19 June 2019, 5 October 2021, 26 July 2022, 10 May 2023, 2 July 2024, 3 September 2025, and 30 September 2026). You confirm that you understand the rules and the counselling process, and that you are submitting original documents and affidavits as required.
You then provide your personal details: NEET PG 2026 roll number, all-India merit rank, full name, parents' or spouse's full name and address, category (unreserved, SC, ST, OBC, EWS), disability status (if applicable), quota (open/NRI/service), mobile number, and email ID.
You then tick boxes confirming you are submitting each of the following documents (see the Documents Required section below for full details).
**Part B – Scrutiny Committee Verification:**
After you submit Part A with your documents, the scrutiny committee at your college will verify that your photo and signature match the NEET PG 2026 data sent by NTA (National Test Agency) to the DME. If there is a match, they will certify this. If there is a discrepancy, they will note it. The committee will also verify that all your documents are genuine and complete. If any document is missing or invalid, they will list the reasons why you are ineligible for admission. The committee chair and members will sign and date the form.
Documents You Must Submit with Proforma-1
The following 14 documents must be submitted along with Proforma-1. Each is mandatory unless marked as conditional (if applicable). Missing even one document can result in rejection of your admission.
- **1. NEET PG 2026 Registration Certificate or Admission Letter**: A photo-bearing copy of your NEET PG 2026 registration or admission letter from NTA.
- **2. Mandatory Internship Completion Certificate**: Original certificate proving you have completed your mandatory internship after MBBS.
- **3. Final MBBS Exam Marks Sheet**: Original or certified copy of your final MBBS exam marks sheet.

Seat Bond Proformas: What You're Committing To
If you are allotted a seat in an MD or MS programme under the state quota (government college), you must sign a seat locking bond (Proforma-2(a) or 2(b)). This is a legal contract between you and the college. Here's what it means:
**Seat Locking Bond Amount**: For MD/MS programmes, the bond amount is ₹30,00,000 (30 lakh rupees). You must deposit 0.5% of this amount (₹15,000) upfront as a security deposit. If you complete the course and comply with all conditions, this deposit is refunded.
**What Triggers the Full Bond Amount**: If you withdraw from the course after the specified time limit (as per the rules) or if you are dismissed for misconduct, you must pay the full ₹30 lakh to the college. The college will not return your original documents if you fail to pay.
**Rural Service Obligations**: In addition to the seat bond, you must sign a rural service bond (Proforma-2(b) or 2(c)). This commits you to serve in rural areas designated by the government after you complete your course. The duration and penalty depend on your employment status: non-service candidates commit to 1 year (₹10 lakh penalty if not served); service candidates (already employed by government) commit to 5 years (₹50 lakh penalty if not served).
**Post-MBBS DNB/PG Diploma Bonds**: If you are allotted a post-MBBS DNB or PG Diploma seat, the bond amount is lower: ₹15,000 for PG Diploma and ₹25,000 for DNB. You deposit 0.5% of the bond value upfront. The same withdrawal and dismissal penalties apply.
NRI Quota Admission: Proformas 3, 4, and 5
If you are an NRI (Non-Resident Indian) or OCI (Overseas Citizen of India) seeking admission under the NRI quota in a private medical college in Madhya Pradesh, you must submit three affidavits, all notarized on ₹1,000 non-judicial stamp paper.
**Proforma-3 (Candidate Affidavit)**: You declare your NRI/OCI status and provide proof. You have three options: (1) submit your OCI card, (2) submit an NRI/OCI status certificate issued by the Indian embassy or consulate general in your country of residence during the admission year, or (3) submit a photocopy of your NRI passport. You also declare whether you are self-sponsored or sponsored by a relative. If sponsored, you list the relationship (parent, grandparent, uncle/aunt, or sibling). You confirm that your sponsor will bear all your educational fees.
**Proforma-4 (Guardian/Sponsor Affidavit)**: Your sponsor (if you are sponsored) must submit this affidavit. The sponsor declares their own NRI/OCI status, provides proof, and confirms they have taken you as their ward. The sponsor lists the relationship and confirms they will pay all your fees for the entire duration of the programme.
**Proforma-5 (NRI/OCI Sponsorship Certificate)**: Your sponsor must also submit this certificate, notarized in their country of residence. The sponsor provides their current address, confirms they have taken you as their ward, and undertakes to bear all your fees until you complete the programme. The sponsor also provides their passport number, country, and date of issue.
⚠️ **CRITICAL TRAP**: All three affidavits must be notarized. A non-notarized affidavit will be rejected. If you are sponsored, both you and your sponsor must submit affidavits; if only one submits, your admission will be rejected.
Proforma-6: The Conduct and Stipend Affidavit
Proforma-6 is a conduct and stipend affidavit that you must submit on ₹200 non-judicial stamp paper. In this affidavit, you declare that:
1. You have read and understood the MP Medical Education Admission Rules 2018 and all amendments.
2. You are aware of the course fees and monthly stipend (if applicable) and agree to comply with them.
3. You will comply with the IMC (Indian Medical Council) Regulation 2002 on professional conduct, etiquette, and ethics. You will not engage in misconduct, ragging, or any unethical activity during the course. You will not be absent without authorization or participate in any strike or agitation.
4. You will comply with all government laws and directives issued from time to time.
You must sign this affidavit in the presence of two witnesses (who must provide their names, addresses, and mobile numbers) and your parent or guardian (who must provide their name, address, mobile number, and email). The affidavit must be dated and signed by you.
How to Fill and Submit These Forms
Every kind of PG seat, one rank
See the real seats your NEET-PG rank can reach.
**Step 1: Download the Forms**: All seven proformas are available on the DME MP website (dme.mponline.gov.in). Download them and print them on the specified paper (non-judicial stamp paper for affidavits, plain paper for other forms).
**Step 2: Fill Proforma-1 (Part A)**: Complete all fields in Part A with your personal details, NEET PG 2026 information, and category/quota details. Tick the boxes confirming you have read the rules and are submitting the required documents.
**Step 3: Gather All Documents**: Collect all 14 documents listed above (or the subset applicable to you). Ensure each document is an original or certified copy as specified.
**Step 4: Fill Conditional Proformas**: Depending on your seat allotment, fill the relevant proformas: Proforma-2(a) and 2(b) for MD/MS state quota; Proforma-2(c) for MD/MS all-India quota; Proforma-2(d) for post-MBBS DNB/PG Diploma; Proformas-3, 4, and 5 for NRI quota.
**Step 5: Get Affidavits Notarized**: For Proformas-3, 4, 5, and 6, you must get them notarized. For Proformas-3, 4, and 5, use ₹1,000 non-judicial stamp paper; for Proforma-6, use ₹200 non-judicial stamp paper. Have a notary public verify and sign the affidavits.
**Step 6: Submit at College**: On the day of admission, submit all completed proformas and documents to the scrutiny committee at your college. The committee will verify each document and sign Part B of Proforma-1.
**Step 7: Retain Copies**: Keep photocopies of all submitted documents and proformas for your records.
Common Mistakes That Get Applications Rejected
**1. Submitting Outdated Category Certificates**: EWS, caste, and income certificates must be issued for the financial year 2025–26 (ending 31 March 2026). Certificates from previous years will be rejected. Check the issue date on every certificate before submitting.
**2. Missing Sub-Caste on OBC Certificate**: If you are claiming OBC category, your caste certificate must clearly show your caste and sub-caste. A certificate that lists only the caste name without sub-caste details may be rejected.
**3. Non-Notarized Affidavits**: Proformas-3, 4, 5, and 6 must be notarized. If you submit them without notarization, they will be rejected. Do not skip this step.
**4. Incomplete NRI Documentation**: If you are an NRI candidate, you must submit all three affidavits (Proformas-3, 4, and 5) AND proof of NRI/OCI status (OCI card, embassy certificate, or passport). Missing any one of these will result in rejection.
**5. Forgetting the Internship Certificate**: Many candidates overlook the mandatory internship completion certificate. Without it, your admission will be rejected, even if all other documents are in order.
**6. Wrong Bond Amount**: Ensure you are signing the correct bond proforma for your seat type. MD/MS bonds are different from DNB/PG Diploma bonds. Signing the wrong form can invalidate your admission.
**7. Incomplete Witness Information on Proforma-6**: The conduct affidavit (Proforma-6) requires two witnesses and your parent/guardian. All must provide their full names, addresses, mobile numbers, and (for parent/guardian) email. Incomplete witness details will result in rejection.
**8. Submitting Photocopies Instead of Originals**: Unless explicitly stated as 'photocopy acceptable', submit original documents. Photocopies of certificates (except ID proofs) are often rejected.
Important Dates and Deadlines
The notice does not specify explicit submission deadlines for these proformas. However, they must be submitted **at the time of admission** — i.e., when you report to your allotted college after receiving your seat allotment. The exact date will be communicated by the DME in the counselling schedule. Check the DME MP website and your email regularly for the admission schedule.
The financial year cutoff for category certificates is **31 March 2026**. Any certificate issued after this date will not be accepted for the 2026–27 admission cycle.

See My CollegesFrequently asked questions
Do I have to submit all seven proformas, or only some of them?
You must submit Proforma-1 (candidate declaration) and Proforma-6 (conduct affidavit) regardless of your seat type. You must also submit the conditional proformas based on your seat allotment: Proformas-2(a) and 2(b) for MD/MS state quota; Proforma-2(c) for MD/MS all-India quota; Proforma-2(d) for post-MBBS DNB/PG Diploma; and Proformas-3, 4, and 5 if you are an NRI candidate. Missing any required proforma will result in rejection of your admission.
What happens if I don't submit the seat bond proforma?
If you are allotted an MD or MS seat and do not submit the seat bond proforma, your admission will be rejected. The seat bond is a legal requirement under the MP Medical Education Admission Rules 2018. Without it, the college cannot finalize your admission.
Can I submit photocopies of my category certificates instead of originals?
No. Category certificates (EWS, SC, ST, OBC, income certificate) must be submitted as originals. Photocopies are not accepted. However, you may submit photocopies of your ID proof (Aadhaar, voter card, passport, etc.). Always check the specific requirement for each document.
My EWS certificate is from 2024–25. Can I use it for admission in 2026–27?
No. The EWS certificate must be issued for the financial year 2025–26 (ending 31 March 2026). A certificate from 2024–25 or any other year will be rejected. You must obtain a fresh certificate for FY 2025–26 from the competent authority in your state.
I am an NRI candidate. Do I need to submit all three affidavits (Proformas-3, 4, and 5)?
If you are self-sponsored, you must submit Proforma-3 (your affidavit) and Proforma-5 (sponsorship certificate). If you are sponsored by a relative, you must submit all three: Proforma-3 (your affidavit), Proforma-4 (your sponsor's affidavit), and Proforma-5 (sponsorship certificate). All three must be notarized on ₹1,000 non-judicial stamp paper.
What is the seat bond amount, and when do I have to pay it?
For MD/MS programmes, the seat bond is ₹30,00,000 (30 lakh rupees). You must deposit 0.5% of this amount (₹15,000) upfront as a security deposit at the time of admission. If you complete the course and comply with all conditions, this deposit is refunded. If you withdraw or are dismissed, you must pay the full ₹30 lakh.
What is the rural service obligation, and what happens if I don't serve?
If you are allotted an MD or MS seat under the state quota, you must commit to rural service after graduation. Non-service candidates commit to 1 year of rural service; if you do not serve, you must pay ₹10 lakh to the government. Service candidates (already employed by government) commit to 5 years of rural service; if you do not serve, you must pay ₹50 lakh. The rural service bond (Proforma-2(b) or 2(c)) is a legal contract.
Can I submit the affidavits without notarization?
No. Proformas-3, 4, 5, and 6 must be notarized. A non-notarized affidavit will be rejected. You must take these forms to a notary public and have them verify and sign the affidavits. For Proformas-3, 4, and 5, use ₹1,000 non-judicial stamp paper; for Proforma-6, use ₹200 non-judicial stamp paper.
What if my OBC certificate does not show my sub-caste?
Your OBC certificate must clearly show your caste and sub-caste. If it does not, it may be rejected. Contact the issuing authority (tahsildar or competent officer) and request a corrected certificate that includes your sub-caste details before submitting it to the college.
When and where do I submit these proformas?
You must submit all proformas and documents at the time of admission — i.e., when you report to your allotted college after receiving your seat allotment. The exact date and time will be communicated by the DME in the counselling schedule. Submit them to the scrutiny committee at your college.
What documents do I need to prove I am an NRI?
You must provide one of the following: (1) your OCI card, (2) an NRI/OCI status certificate issued by the Indian embassy or consulate general in your country of residence during the admission year, or (3) a photocopy of your NRI passport. At least one of these must be submitted with your NRI affidavits.
If I withdraw from the course after admission, what happens to my seat bond deposit?
If you withdraw after the specified time limit or are dismissed for misconduct, you must pay the full seat bond amount (₹30 lakh for MD/MS). Your initial security deposit (₹15,000) will not be refunded, and the college will retain your original documents. The college will not return your documents until the full bond amount is paid.
Download all seven proformas from the DME MP website (dme.mponline.gov.in) immediately. Carefully fill out Proforma-1 and Proforma-6, and the conditional proformas based on your seat type. Gather all 14 required documents, ensuring category certificates are for FY 2025–26 and that affidavits are notarized on the correct stamp paper. Submit everything to the scrutiny committee at your college on the day of admission — missing or incomplete documents will result in rejection of your seat. Keep photocopies of all submitted documents for your records.
