Why this matters — the traps to avoid
Affidavits and certificates are one of the most common reasons PwBD candidates face problems at the counselling or document verification stage. A few points from this notice deserve special attention.
First, the affidavit must be notarized — an unnotarized, unsigned, or self-typed declaration without the prescribed table format will likely be rejected. Second, the UDID Card number and its issuing details must be filled in exactly; a missing or invalid UDID number can cause the whole document to be treated as incomplete. Third, this document must state the diagnosed condition specifically — vague or generic descriptions may not satisfy assessment boards.
