Use this format for achievements not covered by the other three — School Games Federation (SGF) events, Central Government sports schemes, World/Asian school games, and National, Regional or State-level Inter-District games.
This certificate is issued at the district level by the District Educational Officer, District Youth Services Officer, or the Secretary of the SGF.
- Applies to: SGF events, Central Govt. schemes, World/Asian School Games, National/State/Inter-District games
- Issuing authority: District Educational Officer (Govt. of A.P.), DYSO, or Secretary, SGF

Common traps that lead to rejection
Each proforma explicitly notes that it is valid only when signed personally by the named officer. This is the single biggest trap: a certificate that is otherwise complete but signed by a deputy, an assistant, or any office-bearer other than the one specified will not be treated as valid.
