Identity verification and the final fitness declaration

The doctor records the candidate's name, parent's name, and residential address as verified from one of these.

At the end, the doctor gives a written declaration that they could not discover any physical or mental disease that makes the candidate unsuitable to continue studying the UG Medical/Dental/AYUSH course, and then explicitly ticks whether the candidate is FIT or UNFIT to join the institution.
